The Patient Safety Incident Response Framework (PSIRF) is a new approach adopted by healthcare providers to address patient safety incidents. Replacing the previous Serious Incident Framework (SIF), PSIRF represents a fundamental change in how we manage and respond to these incidents.
PSIRF is designed to help build and maintain an effective response system for patient safety incidents, promoting more compassionate engagement and involvement with those affected.
A core element of PSIRF is understanding how incidents occur from a systems perspective. This approach enables us to learn, improve, and create a safer healthcare environment for patients.
The four key objectives of PSIRF are:
- Compassionate engagement and involvement of individuals affected by patient safety incidents.
- Use of various system-based methods for learning from patient safety incidents.
- Proportionate and thoughtful responses to patient safety incidents.
- Oversight that focuses on enhancing the effectiveness and improvement of the response system.
